All-on-4 is an implant method that restores an entire dental arch with a fixed prosthesis supported by four implants. It is intended for patients whose jaw is edentulous, or whose remaining teeth can no longer be saved. The two rear implants are placed at an angle, which makes the most of the available bone and often avoids the need for a separate bone graft.

Unlike a removable denture, an All-on-4 prosthesis does not cover the palate and does not shift while you eat or speak. It is secured to the implants with screws and can only be removed by a dentist.

Who All-on-4 suits

• The upper or lower jaw is edentulous, or the teeth need to be extracted.
• A removable denture will not stay in place, rubs the gums, or interferes with eating and speech.
• Bone volume is reduced at the back of the jaw but preserved at the front.
• You want a fixed solution without a palatal plate.

Materials and fabrication

The implants are titanium — biocompatible and corrosion-resistant. A temporary PMMA prosthesis is made immediately after surgery, so you can eat and smile throughout healing. The final fixed prosthesis is made from zirconia: it holds its colour over the years and withstands chewing loads well.

Treatment steps

• Consultation and diagnostics. Examination, 3D imaging and assessment of bone volume.
• Treatment plan. We agree the implant positions, prosthesis type, schedule and final cost.
• Surgical stage. Unsalvageable teeth are removed and four implants placed in a single visit.
• Temporary prosthesis. Usually fitted within a few days of surgery.
• Healing. Osseointegration takes 3–6 months.
• Final prosthesis. Impressions are taken and the definitive zirconia prosthesis is made.

Care and durability

An All-on-4 prosthesis needs daily care, just like natural teeth. Brush twice a day, clean the space between the prosthesis and the gum with an interdental brush or water flosser, and come in twice a year for professional hygiene. Once a year the dentist checks the implants and the fit of the prosthesis. With good care, implants last for many years.
